MVD REQUEST FOR HEARING FORM

If you have been arrested for a DWI, use this form within ten (10) days of your arrest to request an MVD hearing to attempt to keep your license from being revoked. Make sure to check that you want the officer present and include the $25 check or money order. We also recommend sending this by certified mail with return receipt if you are able.
